Calf pain is common and is often caused by something relatively minor, such as a muscle strain, overuse, cramping, or soreness after exercise. However, pain in one calf can occasionally be a warning sign of a more serious problem, including a deep vein thrombosis (DVT), infection, significant injury, or reduced blood flow to the leg.
The challenge is that a blood clot can sometimes feel similar to an ordinary muscle injury. There is no reliable at-home test that can completely rule out a DVT based on symptoms alone.
If your calf pain is accompanied by new one-sided leg swelling, warmth, redness or discoloration, or unexplained tenderness, especially if you have risk factors for a blood clot, you should seek prompt medical evaluation. If leg symptoms occur with shortness of breath, chest pain, coughing up blood, fainting, or severe breathing difficulty, go to the emergency room or call emergency services because these can be symptoms of a pulmonary embolism.

What Causes Calf Pain?
The calf contains several muscles in the back of the lower leg, particularly the gastrocnemius and soleus muscles. These muscles help you walk, run, climb stairs, stand on your toes, and push your foot downward.
Because the calf is used so frequently, muscular causes of pain are common. However, the lower leg also contains major blood vessels, nerves, tendons, bones, and soft tissues, so calf pain does not always originate from the muscle itself.
Common causes include:
- Calf muscle strain
- Muscle overuse
- Exercise-related soreness
- Muscle cramps
- Minor injuries
- Achilles tendon problems
- Nerve irritation
- Deep vein thrombosis (DVT)
- Cellulitis or another infection
- Reduced arterial blood flow
- Less commonly, fractures, compartment syndrome, or other significant injuries
The circumstances surrounding the pain often provide important clues.
Calf Muscle Strain: A Common Cause of Calf Pain
A calf strain occurs when muscle fibers are overstretched or torn.
This commonly happens during running, jumping, sports, sudden acceleration, climbing, or other activities requiring forceful contraction of the calf muscles.
A calf strain is more likely when pain:
- Started during or shortly after physical activity
- Followed a sudden push-off, jump, sprint, or awkward movement
- Is reproduced by using or stretching the calf muscle
- Is localized to a particular area of the muscle
- Improves with rest
- Is associated with muscle tenderness
- May be accompanied by bruising after a more significant injury
Some people describe feeling a sudden pulling, tearing, or popping sensation.
Mild strains may simply feel sore or tight. More substantial strains can cause significant pain and difficulty walking.
How Long Does a Calf Strain Take to Heal?
Recovery depends on the severity of the injury.
A mild strain may begin improving over several days and recover over approximately one to a few weeks. More substantial muscle injuries can require several weeks or longer.
Persistent pain, inability to bear weight, substantial bruising or swelling, worsening symptoms, or failure to improve as expected should prompt medical evaluation.
Calf Pain From Muscle Cramps
A muscle cramp is a sudden involuntary contraction of the muscle.
Calf cramps may occur during exercise or at night and can cause intense but temporary pain.
Potential contributors include:
- Prolonged or intense exercise
- Muscle fatigue
- Prolonged positioning
- Certain medications
- Fluid or electrolyte disturbances in some situations
After a severe cramp resolves, the calf may remain sore for some time.
Recurring or unexplained cramps should not automatically be attributed to dehydration. If cramps are frequent, severe, or accompanied by weakness, swelling, numbness, medication changes, or other symptoms, discuss them with a healthcare professional.
When Could Calf Pain Be a Blood Clot?
One of the most important conditions to consider with unexplained calf pain is deep vein thrombosis (DVT).
A DVT occurs when a blood clot develops in a deep vein, most commonly in the leg.
The primary danger is that part of the clot can break loose and travel through the bloodstream to the lungs. This is called a pulmonary embolism (PE) and can be life-threatening.
Possible symptoms of DVT include:
- Pain or tenderness in one leg
- New swelling of one leg
- Calf swelling
- Warmth
- Red or discolored skin
- A sensation of tightness or heaviness
- Unexplained symptoms without a clear muscular injury
However, DVT can sometimes cause only mild symptoms, and some people have few noticeable symptoms.
This is why the absence of dramatic swelling or redness does not reliably rule out a blood clot.
Calf Strain vs Blood Clot: What’s the Difference?
There is overlap between the two conditions, so no symptom checklist can diagnose or exclude DVT with certainty.
However, some features can make one possibility more likely than another.

The important point is that these findings overlap.
For example, a DVT may cause calf tenderness that feels muscular. Conversely, a significant muscle strain can produce swelling and warmth.
If DVT is clinically suspected, the distinction may require medical evaluation and diagnostic testing.
Does Calf Pain in One Leg Mean a Blood Clot?
No. One-sided calf pain has many possible causes and does not automatically mean you have a DVT.
A muscle strain, tendon injury, nerve irritation, trauma, infection, or other musculoskeletal problem can affect only one leg.
However, new unexplained pain and swelling involving only one leg deserves greater attention, particularly when there is no obvious injury or when DVT risk factors are present.
The combination of symptoms and medical history matters more than any single finding.
Risk Factors for DVT
Your healthcare provider will consider your risk factors when evaluating unexplained calf pain or swelling.
Factors that may increase the likelihood of venous thrombosis include:
- Previous DVT or pulmonary embolism
- Recent major surgery
- Recent hospitalization
- Prolonged immobility
- Being confined to bed
- Certain cancers
- Pregnancy and the postpartum period
- Estrogen-containing medications in appropriate contexts
- Certain blood-clotting disorders
- Significant trauma
- Increasing age
- Other medical conditions that increase thrombosis risk
Long periods of limited movement, such as certain long-distance trips, may also contribute to risk, particularly when combined with other factors.
Having a risk factor does not mean that calf pain is definitely a blood clot. Likewise, having no obvious risk factors does not make DVT impossible.
Can You Check for a Blood Clot at Home?
There is no dependable home maneuver that can safely rule out a DVT.
You may encounter advice online suggesting that pulling the foot upward and checking for calf pain can diagnose a blood clot. This is associated with the historical Homan sign.
Homan sign is not sufficiently reliable for diagnosing or excluding DVT and should not be used to decide whether medical evaluation is necessary.
Do not repeatedly squeeze, massage, or manipulate a painful swollen leg in an attempt to determine whether a clot is present.
If your symptoms and risk factors raise concern for DVT, seek professional evaluation.
How Is a Possible DVT Evaluated?
A healthcare professional usually begins with a detailed history and examination.
Important questions may include:
- When did the pain begin?
- Was there an injury?
- Is one leg more swollen than the other?
- Have you recently had surgery?
- Have you recently been hospitalized?
- Have you had a previous blood clot?
- Have you experienced prolonged immobility?
- Are you pregnant or recently postpartum?
- Do you have active cancer or other important risk factors?
- Are you experiencing chest pain or difficulty breathing?
The clinician may examine both legs for swelling, tenderness, warmth, skin changes, pulses, range of motion, and other abnormalities.
Clinical decision tools may also be used to estimate the probability of DVT.
Depending on the person’s risk and presentation, further testing can include a D-dimer blood test and/or compression ultrasonography of the leg veins.
A physical examination alone generally cannot definitively exclude DVT when clinical suspicion is significant.
Can Urgent Care Diagnose a Blood Clot?
It depends on the facility.
An urgent care clinic can evaluate your symptoms, assess risk factors, examine your leg, and determine how urgently further testing is needed.
However, many urgent care centers do not have immediate access to vascular ultrasound or other diagnostic resources needed to evaluate a suspected DVT.
If the clinician believes a blood clot needs to be excluded and the required testing is unavailable, you may be referred to an emergency department or another facility capable of performing the appropriate testing.
This is one reason it is helpful to call an urgent care center beforehand if you specifically need evaluation for a possible blood clot.
Other Serious Causes of Calf Pain
A blood clot is not the only potentially serious cause of calf pain.
Cellulitis
Cellulitis is a bacterial infection of the deeper layers of the skin and surrounding tissues.
Symptoms may include:
- Redness
- Warmth
- Swelling
- Pain or tenderness
- Expanding skin changes
- Fever or chills in some cases
A skin break, wound, ulcer, insect bite, or other disruption of the skin can sometimes precede the infection.
Rapidly spreading redness, severe pain, systemic illness, or other concerning findings require prompt medical attention.
Reduced Arterial Blood Flow
Arteries deliver oxygen-rich blood to the legs.
Chronic narrowing of these arteries, known as peripheral artery disease (PAD), can cause leg discomfort with activity that improves with rest.
A sudden severe reduction in blood supply is much more urgent.
Emergency warning signs can include sudden severe leg or foot pain, unusual paleness, coldness, numbness, weakness, or difficulty detecting a pulse.
Sudden loss of arterial blood flow is an emergency.
Compartment Syndrome
Acute compartment syndrome occurs when pressure rises within a confined muscle compartment, compromising circulation and tissue function.
It is more commonly associated with significant injuries, although other causes exist.
Warning findings may include:
- Severe and progressively worsening pain
- Pain that seems disproportionate to the apparent injury
- A tense or very swollen extremity
- Numbness or tingling
- Weakness
- Severe pain with certain movements or stretching
Suspected acute compartment syndrome requires emergency evaluation.
Achilles Tendon Injury
The Achilles tendon connects the calf muscles to the heel.
An Achilles injury can cause pain in the lower calf or back of the ankle. A rupture may occur during sudden acceleration, jumping, or sports.
Some people describe feeling as though they were kicked in the back of the ankle despite nobody touching them.
Difficulty pushing off while walking or standing on the toes can occur.
A suspected rupture requires medical evaluation.
When Should You Go to Urgent Care for Calf Pain?
Urgent care may be appropriate for calf pain when you are otherwise stable but need evaluation for symptoms such as:
- Persistent calf pain without a clear explanation
- Pain after a minor injury
- Suspected muscle strain
- Mild-to-moderate swelling
- Difficulty walking because of pain
- Persistent symptoms that are not improving
- Possible cellulitis without severe systemic symptoms
However, if DVT is a significant possibility, the urgent care clinician may need to refer you elsewhere for vascular ultrasound or emergency evaluation.
Availability of imaging varies considerably between urgent care centers.
When Should Calf Pain Go to the ER?
Seek emergency medical evaluation for calf or leg symptoms associated with potentially serious complications.
Examples include:
- New calf/leg symptoms accompanied by chest pain
- New or unexplained shortness of breath
- Coughing up blood
- Fainting or near-fainting
- Severe difficulty breathing
- Sudden severe leg pain
- A suddenly cold, pale, numb, or weak foot or leg
- Severe rapidly increasing swelling or pain
- Major trauma or obvious deformity
- Severe pain that seems disproportionate to the injury
Chest pain or difficulty breathing in someone with possible DVT symptoms is particularly concerning because a clot may have traveled to the lungs.
Do not delay emergency care in that situation.
What Is a Pulmonary Embolism?
A pulmonary embolism occurs when material—most commonly a blood clot originating in a deep vein—travels to the pulmonary arteries in the lungs.
Symptoms can include:
- Sudden shortness of breath
- Chest pain, sometimes worse with breathing
- Rapid heart rate
- Lightheadedness
- Fainting
- Cough
- Coughing up blood
- Low oxygen levels
- Severe weakness or distress
Symptoms vary, and not every patient experiences all of these findings.
A suspected pulmonary embolism requires emergency evaluation.
What If My Calf Hurts After Exercise?
Calf pain after exercise is commonly musculoskeletal, especially if:
- You recently increased your activity
- You performed unfamiliar exercises
- The discomfort involves the muscle used during exercise
- There was a clear strain or overuse mechanism
- Symptoms gradually improve with rest
For a mild uncomplicated muscle strain, initial treatment may include temporarily reducing the activity that provoked pain and gradually returning to normal activity as symptoms improve.
Ice may help during the early phase of an acute injury. Depending on the injury and individual medical history, over-the-counter pain medications may sometimes be appropriate.
However, NSAIDs such as ibuprofen or naproxen are not appropriate for everyone. People with certain kidney problems, gastrointestinal bleeding or ulcers, anticoagulant use, medication interactions, pregnancy considerations, or other medical conditions should ask a healthcare professional before using them.
Exercise should not automatically be assumed to explain new unilateral swelling, substantial warmth/redness, or otherwise unexplained calf symptoms.
Should You Massage a Painful Calf?
Gentle massage may sometimes feel helpful for uncomplicated muscular soreness.
However, if you have unexplained one-sided swelling and pain and are concerned about a blood clot, do not rely on massage as treatment or use aggressive manipulation while delaying evaluation.
The priority is determining why the leg is painful or swollen.
Can Calf Pain Happen Without Swelling?
Yes.
Many muscular and orthopedic conditions can cause calf pain without visible swelling.
A DVT can also occur without dramatic swelling, so the absence of swelling does not independently rule it out.
Healthcare professionals consider the entire clinical picture rather than relying on a single symptom.
Can You Have a Blood Clot Without Redness?
Yes.
Redness and warmth can occur with DVT, but neither is required.
A person can have a DVT without obvious redness, which is another reason symptoms alone are insufficient to exclude the diagnosis.
Can a Blood Clot Feel Like a Pulled Muscle?
It can.
Both DVT and muscular injuries may cause calf tenderness or aching.
A clear injury mechanism and pain specifically associated with muscle activity may favor a musculoskeletal explanation, while unexplained unilateral swelling and important thrombosis risk factors may increase concern for DVT.
But there is enough overlap that a suspected blood clot should not be diagnosed—or dismissed—solely by how the pain feels.
What If the Calf Is Red, Warm, and Swollen?
A red, warm, swollen calf deserves medical attention because several conditions can cause this combination.
Important possibilities include:
- Deep vein thrombosis
- Cellulitis
- Injury or inflammation
- Other vascular or soft-tissue conditions
Fever, rapidly spreading redness, wounds, drainage, or systemic symptoms may increase concern for infection.
Risk factors for thrombosis or otherwise unexplained one-sided swelling may increase concern for DVT.
Sometimes the distinction requires testing.
What If Both Calves or Legs Are Swollen?
Swelling involving both legs has a different differential diagnosis from sudden swelling confined to one leg.
Possible contributors include prolonged standing, venous insufficiency, medication effects, and fluid retention related to systemic medical conditions.
Heart, kidney, liver, or vascular problems can contribute to bilateral edema in some patients.
New or worsening swelling in both legs should be evaluated, particularly when associated with:
- Shortness of breath
- Difficulty breathing while lying flat
- Chest symptoms
- Rapid weight gain
- Reduced urine output
- Significant weakness
- Other new systemic symptoms

How Is Calf Pain Treated?
Treatment depends entirely on the underlying cause.
Muscle strain
Treatment may include activity modification, appropriate use of ice or other supportive measures, gradual rehabilitation, and pain control when medically appropriate.
Deep vein thrombosis
DVT commonly requires anticoagulation or other management determined by a healthcare professional based on the patient’s individual situation.
A suspected DVT should not be treated at home as though it were merely a muscle strain without appropriate evaluation.
Cellulitis
Bacterial cellulitis generally requires appropriate antibiotic treatment. The medication and route depend on severity, location, patient factors, and the suspected organism.
Arterial problems
Significant arterial insufficiency requires medical evaluation, and acute loss of circulation is an emergency.
Significant tendon or muscle injury
More substantial injuries may require immobilization, rehabilitation, orthopedic or sports-medicine evaluation, and occasionally surgical treatment.
The correct treatment therefore depends on establishing the likely cause.
When Should You Worry About Calf Pain?
Most calf pain is not caused by a life-threatening condition.
You should be more concerned when the pain is:
- Unexplained
- Predominantly in one leg
- Associated with new swelling
- Accompanied by warmth or skin discoloration
- Occurring after significant immobility, surgery, hospitalization, or another important DVT risk factor
- Progressively worsening
- Associated with significant infection symptoms
- Associated with circulation changes in the foot
The combination of calf symptoms plus chest pain, shortness of breath, fainting, or coughing up blood requires emergency evaluation.
